Adenomyosis: Symptoms, Causes, IVF Success and More
The uterus is well-defined by an inner covering called the endometrium or the uterine lining. The muscular layer called the myometrium lies beneath the endometrium. During regular menstruation, the uterine lining thickens, sheds and bleeds.
Adenomyosis of the uterus is when the endometrium grows invading the myometrium of the uterus. When the uterine lining bleeds during menstruations, the endometrium within the muscles too breakdown; rupturing the muscular wall. Thus, adenomyosis is characterized by heavy bleeding, severe muscle cramps, and an enlarged uterus.
Studies suggest that women in their reproductive age, women who have had a previous uterus surgery or c-section, can develop adenomyosis symptoms. There are many theories to explain the cause of adenomyosis development.
One theory suggests that the endometrium is deposited in the uterine muscular wall during fetal development.
Another theory indicates that bone marrow intrusions might have caused adenomyosis.
Some experts even believe that the inflammation of uterine lining during childbirth might have lead to such endometrial invasions causing adenomyosis
While contribution of imaging techniques such as Ultrasound, Magnetic Resonance Imaging (MRI) has led to a more accurate diagnosis of adenomyosis it is still unclear on how adenomyosis is related to fertility issues. However, certain studies indicate that women with 27-70% endometriosis are likely to have adenomyosis. Since endometriosis causes fertility problems, adenomyosis may indirectly lead to infertility.
In Vitro fertilization (IVF) is one of the most successful procedures that has helped many couples overcome fertility concerns. But, several studies have shown that adenomyosis can impact adenomyosis IVF success rate. In other words, adenomyosis can impact a successful clinical pregnancy and increase the likelihood of a pregnancy loss before term even after an IVF embryo transfer. This may be because of the problems relating to the uterus which may not be able to provide favourable uterine conditions for a healthy conception.
Treatment of adenomyosis is all about management of the symptoms such as menstrual pain and excessive bleeding. Over-the-counter medications, pain relievers, hormonal therapies are some of the treatment options that can be considered. It is observed that the adenomyosis clinical features fade away after menopause. But when nothing works much, the last resort to relieve adenomyosis discomfort can be a surgery to remove the uterus permanently (Hysterectomy).
