IUI vs IVF : Which fertility Treatment is Right For You?

If you’re thinking about getting an infertility treatment, you might have come across the treatments Intrauterine Insemination or IUI and In-vitro Fertilisation or IVF. Both these treatments are used by fertility doctors depending upon what is the cause of infertility.

Before we understand the IUI vs IVF it is important to know what these treatments are and how they are different from each other.

For a pregnancy to happen, the ovum or the female’s egg that is released from the ovaries needs to fertilize with the male sperm in the fallopian tube of the female reproductive tract to produce an embryo which implants in the uterine lining to form a pregnancy. When this happens naturally, the male sperm has to travel from the vagina to the fallopian tube and fertilize with an ovum to form a viable pregnancy.

IUI involves putting the sperm into the woman’s uterus with the help of a soft catheter. This is synced with the ovulation of the woman to help improve the fertilisation of the egg and the sperm. IUI also helps the sperm travel to the ovum faster, as they have to travel to the fallopian tube from the uterus instead of the vagina. Once fertilisation of the egg and the sperm occur and an embryo is produced it can implant itself in the uterus like in a natural pregnancy. IUI is a simple procedure and is much less costly when compared to IVF.

IVF has different procedures which help with conceiving. In IVF the egg is retrieved from the ovaries, after ovarian stimulation that helps maturing of multiple eggs. These eggs are then prepared and mixed with the sperm in a lab equipment to help the egg and sperm fertilize. Once fertilized and formed into a healthy embryo, one or more embryos are transferred which are then implanted into the uterus. Embryos can also be frozen to be transferred in another cycle, as it is preferred with people having PCOS, or they can be frozen for use in a later cycle if one cycle of IVF fails and subsequent cycles require frozen embryos. IVF treatment procedure can also involve some more procedures if there are any complications with the egg, sperm, uterus or fallopian tubes. IVF is an expensive procedure but can help with a wider range of infertility issues than IUI making it a better option for some people facing infertility issues.

Difference between IUI vs IVF

There are many differences between IUI  vs IVF. It is important the right treatment is provided to you for your requirements for a successful infertility treatment. The following are the differences between the IUI treatment and the IVF treatment –

·   Invasiveness – IUI is a less invasive procedure when compared to IVF since in IUI the sperm is placed into the uterus using a soft catheter, whereas in IVF the eggs are invasively taken from the ovaries.

· Effectiveness – IUI may not be effective in the first cycle and multiple cycles may be required. IUI also may not be effective in some conditions where an IVF treatment would work better. IUI is useful when the female egg reserves, uterine health, fallopian tubes and egg quality are good and the sperm quality, morphology, motility and concentration is suitable for an IUI treatment.

·   Cost – IUI is much cheaper when compared to the IVF procedure.

·  Complexity – IVF is a much more complex fertility treatment since it requires multiple procedures such as ovarian stimulation, egg retrieval, sperm and egg preparation, fertilization and embryo transfer. Additional treatments such as PESA, TESA, hysteroscopy, and laparoscopy may be required if any complications are to be treated during fertility treatment.

IVF or IUI – Which fertility treatment is right for you?

To know whether you should go for IVF or IUI, your doctor will do a consultation and see what process you can benefit better from for your infertility treatment.

Some of the factors to consider are –

·    Age of the people trying to conceive – Age is a very important consideration as a person who is younger can try for IUI first and then go for IVF if IUI fails since IUI is a simpler procedure.

·    Reason for Infertility – IUI wouldn’t work for every infertility reason, for e.g if the fallopian tubes are blocked then IUI wouldn’t be able to fertilize with the egg. A surgery for removal of the blockage in the tubes followed by an IUI later may be recommended, or your doctor may recommend IVF if they feel that it would be a better approach. Similarly, in people who are having unexplained fertility IUI may be preferred before IVF.

·   Male Infertility – Male infertility is a common condition where IUI might be preferred before going for IVF since if there is a lesser concentration of the sperm in the ejaculate, an IUI can help it reach the fallopian tubes more easily. However if the concentration is below the concentration that is required for an IUI, IVF may be a better approach combined with ICSI (Intracytoplasmic Sperm Injection) as the sperm would be directly injected into the ovum in a lab and the embryo is transferred into the uterus. If there are other male infertility problems such as ejaculation related problems then also IUI can be beneficial.

·   Antisperm Antibodies in cervical mucus – Some women can produce anti-sperm antibodies which are present in the cervical mucus. IUI places the sperm into the uterus thereby preventing this from happening thereby letting the sperm travel to the ovum.
