What is ICSI in IVF Pregnancy?
Intracytoplasmic Sperm injection (ICSI) is an advanced procedure of In Vitro Fertilization (IVF) that can help address major male fertility factors and certain female fertility issues. ICSI can be either performed as an individual procedure or can be an extended procedure of IVF. Intracytoplasmic morphologically selected sperm injection (IMSI) is another extended procedure of IVF and ICSI that allows morphological selection of sperm for the fertilization procedure.
The basic idea of IVF is to perform the fertilization process in-vitro. In a normal fertilization procedure of IVF, the eggs are retrieved from the mature follicles and allowed to fuse with the extracted sperm in an incubated condition. However, sometimes this fusion may not occur precisely leading to poor quality embryos or even failure of fertilization. This problem may be because of varied reasons:
Low sperm count (oligospermia), less motile sperms, or poor sperm morphology
When the sperm numbers are normal, fertilization failure can be due to the production of less mature eggs.
ICSI helps overcome all the problems causing an IVF cycle failure due to fertilization problems or even unexplained infertility where the reason for infertility is not known.
The Procedure of ICSI/IMSI:
ICSI reproduction begins after the retrieval of the egg and sperm from the couple.
The egg is then placed in a petri dish and held with the help of a pipette.
The process of injecting sperm into egg is performed with the help of a sharp hollow needle. The needle pierces into the hard shell of the egg to accomplish the fertilization procedure.
IMSI, also called “Super ICSI” requires a high-resolution microscope that magnifies the sperm 6,300 times. The sperms with proper morphological features such as shape, size, and motility are selected with the help of such a high power magnifying tool for the ICSI or IVF procedures. The embryos developed as a result of IMSI/ICSI fertilization, are transferred into a woman’s uterus for implantation and commencement of pregnancy.

One of the major advantages of ICSI conception is that good-quality embryos can be produced and selected, avoiding the possibility of multiple pregnancies.
Preimplantation genetic diagnosis/screening (PGD/PGS) of the embryos requires the use of ICSI/IMSI that can enable efficient embryo screening.
ICSI/IMSI has proved to improve the quality of embryos because of its effective gamete selection process and precise fertilization technique.
